You will receive new patient forms electronically. Completing them as soon as you receive them helps the team review your information and makes check-in quicker. If you have recent X-rays from another office, you can email them ahead of time.
What should I bring to my dental appointment?
Please bring any oral appliances you currently use, such as dentures, a nightguard, or retainers, so our doctors can check the fit and condition. Many patients also bring a light jacket or blanket since the office may feel cool due to increased air circulation.
