We are honored that you have chosen our office for your dental care in Fremont, California. Our goal is to make your first visit comfortable, organized, and stress-free.

To help us prepare, new patient forms will be sent to you electronically. We kindly ask that you complete these forms as soon as you receive them. This allows our team to review your medical and dental history in advance and helps make your check-in process smooth and efficient.

• If you currently use any oral appliances, such as dentures, a nightguard, or orthodontic retainers, please bring them with you so Dr. Anna Yi can evaluate them.
• The office maintains strong air circulation, so you may wish to bring a light sweater or blanket if you tend to feel cold.
• For the most accurate evaluation and comfortable cleaning, please brush your teeth before your visit, if possible.
• If you have had dental X-rays taken at another office recently, please have them emailed to us at care@fremontfamilysmiles.com prior to your appointment.
